These are the forms which ensured safety of the photo shoot task. This is done by stating each possible risk that could happen to anyone involved, and then saying how this could have been measure to make sure it doesn't happen. As well as ensuring safety, it also condones the fact that the subject in the photo is allowed to be photographed, and also that I am allowed to use the Photo's that I have taken for the final product which I am creating.
